CHEZ- Supported Independent Living (SIL)
What we provide
Chezliving is committed to addressing the complex needs of our residents through a holistic, client-centered approach. Our team of experienced professionals is trained in trauma-informed care, cultural sensitivity, and crisis intervention, ensuring that our services are tailored to the unique experiences of each individual. We believe in the power of community and work diligently to foster social connections, reduce isolation, and promote overall well-being. The CHEZ SIL program offers a community-based program that supports youth and adults in preparing for independence and productive living in the community. Residents live in their own apartment in the community and as they increase their ability to live independently, there is a gradual withdrawal of support services.
We understand that every individual is distinct, and so are their needs. Our client-driven approach ensures that each youth receives tailored support to meet their specific requirements. As they move into their own apartments within the community, we provide a gradual withdrawal of support services, allowing them to develop a strong sense of responsibility for their homes.
We prioritize the dignity of risk and respect the right of each individual to make their own choices. Our goal is to foster an environment where residents can gain awareness and confidence in their ability to navigate the challenges of independent living. A Key features of our program include regular meetings with a dedicated Placement Coordinator and Care planning. This experienced professional provides ongoing support to enhance knowledge in various essential areas, including finance, nutrition, budget-friendly shopping, cooking, home maintenance, personal wellness, problem-solving, conflict resolution, communication and relationship skills, employment, education, career search, and the development of robust support networks.
Whether some individuals require minimal supports or others need ongoing assistance, ranging from case management to assertive community treatment, our comprehensive approach ensures that each resident receives the necessary tools to thrive on their path to independence.
